Are you due for a vaccination or general screening, such as a mammogram, prostate or colon cancer screening or a blood test?

Perhaps you have a specific health issue you’d like to address? It is always a good idea to write down any changes you’ve noticed so you can be sure to talk with your doctor about your concerns.
